1.0 Identifying Information

2.0 Executive Summary

Present a summary assessment of the status of the project after a period of implementation. This includes an over-all assessment of the project in relation to the accomplishment of objectives, fund usage, and compliance with the Terms and Conditions attached to the project.

3.0 Accomplishment of Project Objectives

This presents a detailed assessment of the proponent's accomplishment of objectives set for the period. This section answers two basic questions: (1) where are you now as far as planned activities and target outputs are concerned and (2) problems encountered in project implementation and action taken/to be taken. Please use the table format provided.

4.0 Status of Project Finances

This presents a detailed assessment of the financial performance of the project, with explanation for the variances. This includes an analysis of expenditures versus the activities conducted and objectives achieved so far, e.g., when 80% of the budget has been expended while outputs remain only at 10%, an explanation is necessary.

The financial report must also include the status of counterpart contribution.

5.0 Problems Encountered and Actions Taken/To be Taken

This is a summary listing of the problems encountered in project implementation identified in Parts III and IV. For each and every identified problem, action taken or to be taken must be specified.

6.0 Appendix

Attach to the report the following:

  1. Expenditure and Budget Performance Report;
  2. Counterpart Contribution Report;
  3. Statement of Assets, Liabilities, and Fund Balances; and
  4. Statement of Revenue, Expenses, and Changes in Fund Balance.
