1.0 Summary

1.1 Identifying Information

Project Title: The title should clearly state what the proponent intends to do.

Project Site: This should state the exact location of the project and not the proponent's address. Please check the appropriate island grouping, region, and province.

Beneficiary Group: Kindly indicate the sectors, including the number, that will directly benefit from the project. Likewise, the sectors, including the number, that will indirectly benefit from the project. Check the appropriate box and indicate beside it the estimated number. Direct beneficiaries refer to the target clientele of the project or those that the project intends to assist. Indirect beneficiaries refer to sectors that would also benefit from the project although in a less straightforward manner.

Proponent Organization: Please fill in all the information needed (name, address, telephone number, mobile phone number, fax number, e-mail address).

Contact Person: Kindly provide names including their designation of two persons authorized to sign checks and represent the proponent.

2.0 Project Summary

Please provide a brief description of your project here, including what the project intends to do and its expected impact to its target beneficiaries. Be as specific as possible about what your project intends to accomplish.. Do not the tell the whole story about the project, just provide an overview of what the proposal is all about.

3.0 The Project Context

3.1 Community Profile

In this section, the proponent should provide an overview of the municipality and/or district where the project will be implemented. Data that are relevant to the project (e.g., socio-economic profile of the town and neighboring municipalities such as income class, population growth, population of beneficiaries to be assisted, land use, agricultural area by crop, etc.) should be provided. Secondary research is necessary in order to provide a basic profile of the community.

3.2 Problem Statement/Needs Analysis

Indicate the problem that the project wants to address. Discuss how it became a problem and why the problem persists.

3.3 Beneficiary Groups Affected

Describe the sector/population affected by the problem, e.g., number of men and women, socio-economic status, occupation, and income level.

3.4 Previous/Ongoing efforts to respond to the problem

Describe efforts previously undertaken by the various stakeholders in response to the above problems (if there are any). Discuss specific results of these efforts and lessons learned.
